In that time, it has generated more confusion โ and more controversy โ than almost any other quality measurement in American healthcare. I've spent years sitting with hospital administrators, reviewing the CMS methodology documents, and analyzing how the ratings translate into real patient outcomes. Here's what you need to know.
What CMS Stars Actually Measure
The CMS star rating is a composite of seven quality "groups" collapsed into a single 1โ5 star score using a statistical method called latent variable modeling. The seven groups are:

- Mortality (22% weight) โ 30-day risk-adjusted death rates for heart attack, heart failure, pneumonia, COPD, hip/knee replacement, and CABG

- Readmission (22%) โ Risk-adjusted 30-day readmission rates for seven conditions
- Patient Experience (22%) โ HCAHPS survey scores across ten dimensions
- Effectiveness of Care (4%) โ Process measures: are patients receiving evidence-based treatments?
- Timeliness of Care (4%) โ ED wait times, timely administration of heart attack treatments
- Efficient Use of Medical Imaging (4%) โ Appropriate use of CT scans and MRIs
The Critical Flaw Most Patients Don't Know
Here's the thing that genuinely frustrates me about how CMS stars are presented to the public: the same star score can be reached by completely different performance profiles. A hospital might earn 4 stars by excelling in patient experience while having average mortality and safety scores. Another hospital might earn 4 stars by having excellent clinical outcomes but mediocre patient survey results.

Why Small and Rural Hospitals Often Have More Stars
This is counterintuitive to most patients, but small critical access hospitals often display more stars than large academic medical centers. The reason: large teaching hospitals accept the sickest patients โ complex cases transferred from smaller facilities, high-risk surgeries, rare diseases. Even with excellent care, their risk-adjusted mortality rates are higher simply because their patient population is more severely ill.
The risk adjustment methodology attempts to control for this, but it's imperfect. I consistently advise patients planning complex surgeries to look past star ratings toward condition-specific outcome data from high-volume specialty centers.
When to Trust Stars โ and When Not To
Stars are reasonably reliable for: comparing hospitals of similar size and type for routine care; identifying obviously poor performers (1-star hospitals consistently underperform across most domains); general hospital shopping for non-complex admissions.
Stars are less reliable for: complex, high-risk procedures where volume and specialty experience matter more; comparing large academic centers against community hospitals; facilities that have undergone significant leadership or ownership changes since the data collection period.
